The Community Health and Wellbeing Worker will speak to households about health and wellbeing, alongside offering support to connect people to services and activities in their local community.

We are looking for the right people who can empower residents to take control of their lives. This will involve working with residents in their homes who may be experiencing a number of issues, so you will need outstanding listening and communication skills, empathy, problem solving and enthusiasm. You will need excellent organisational skills, be adept at seeking out information for relevant community services and have the tenacity to help people achieve sustainable improvements in their wellbeing.

Main duties include:

· Managing a caseload of households.

· Providing monthly home visits and providing a continuous point of contact for all members of the household.

· Working with the residents to assess their needs, create personalised action plans and locate support within the community.

· Supporting residents to prioritise and address practical, health, social and emotional issues in their lives which affect their wellbeing.

Healthwatch Kensington & Chelsea

Healthwatch was set up in 2013 to champion the rights of health and social care users, and hold the health and social care system to account for how well it engages with the public. Its remit covers all publicly funded health and social care services for adults and children.

About the role

The role is to lead the work of Healthwatch Kensington & Chelsea, working with a small skilled staff team and a range of volunteers and other community stakeholders. In particular, you will share our commitment to embed user engagement and community development at the heart of the Healthwatch service model, empowering user voices and improving access to health services. 

You’ll have an opportunity to develop the service in line with our ambitious vision. This will include leading on the planning and delivery of targeted projects, supporting a diverse and varied engagement programme, and building relationships with a range of strategic partners. The Healthwatch Manager (Kensington & Chelsea) also takes the lead on managing our local Advisory Board.  You will work closely with the Healthwatch teams in Westminster and Brent. 

Key responsibilities

 Ensure Healthwatch Kensington & Chelsea’s work conforms to all required principles, objectives and statutory obligations, and meets all contractual requirements between The Advocacy Project and Brent Council. 

 Manage and motivate a small staff team to deliver an effective and vibrant local Healthwatch.

 Provide leadership to make sure the patient and public voice is heard across health, social care, children’s and other sectors in a continuously changing external environment. 

 Support the effective working of the Healthwatch Kensington & Chelsea Advisory Board. 

 Ensure local communities are able to participate fully in our work, particularly those that are least likely to have had their voices listened to. Support these communities in influencing local and national policy. 

 Represent Healthwatch Kensington & Chelsea at strategic governance and external meetings with key stakeholders, liaising with nominated Board leads regularly. 

 Develop the service to respond to emerging trends and needs, keeping abreast of Healthwatch England best practice to continually enhance the quality of the service. 

 Monitor the service to make sure it delivers to time, budget and quality; provide reports evidencing this to key stakeholders, including local commissioners, Healthwatch Advisory Board, the Bi-borough Health and Wellbeing Board and Healthwatch England. 

 Develop effective partnerships with key staff in health and social care services. 

 Work with the communications and engagement lead to make sure all relevant internal and external stakeholders receive relevant, timely and up-to-date communication about all Healthwatch projects. 

 Work with the Healthwatch Service Manager and Head of Business Development to identify opportunities for funding additional work which will add value to our Healthwatch services.

Job Purpose

Money Minded: Professional advice and support because your mental health matters

Money Minded is a new service designed to support adults, (18 years and over) in Ealing, Hammersmith & Fulham, Hounslow. The service provides support with welfare benefits, accessing debt support, housing and well-being/peer support groups/workshops. 

The role of the Mental Health Information and Advice Officer will be to provide advice, guidance and customer representation to tri-borough Residents living with Mental Health difficulties. Advice issues may include, housing, benefits, council tax, accessing debt support, signposting etc. 

The successful candidate will deliver support to enquirers and manage a caseload of individuals, as agreed with the Service Manager. In addition to advice casework, the Mental Health Information and Advice Officer will also be expected to assist onboarding referrals to the service, as and when needed. 

The successful candidate will be required to work alongside the Team Leader and volunteers supporting on the project, within the Advice Quality Standard framework (AQS) and adhere to Mind’s policies and procedures. 

Due to the nature of the work, if successful, you will need to have an up-to-date DBS check completed, prior to undertaking unsupervised client work.

We are looking for social media officer to support the Health Equals team to deliver an exciting programme of campaigning work.

The post holder will play a crucial role in helping Health Equals to build its online presence and following, supporting its objective to raise awareness of how our health is shaped by the building blocks of health. South East London Mind is working with Oxleas NHS Foundation Trust and Bridge Support to deliver the Greenwich Mental Health Hub.  This will form part of the new transformation of mental health services in the Royal Borough of Greenwich under the NHS Long Term Plan. The service will bring together the expertise of local primary, secondary and voluntary sector mental health care providers with the aim of providing flexible, holistic, and integrated services for adults with mental health problems.

Unpaid carers in our communities are supporting relatives, friends and neighbours, often with limited or no support, to live the best lives they can.  The support of people with dementia, learning or physical disabilities, mental health difficulties or substance misuse problems by unpaid carers has been estimated to save the government close to the equivalent expenditure for the whole NHS.

We are looking for a Mental Health Carers Advisor who possesses an understanding of the challenges of unpaid caring and whose experience of working within mental health services includes support, prevention, recovery, and self-management. Applicants should possess effective skills in communication and have experience of completing assessments, managing a diverse caseload and working in a busy work environment. An ability to work independently but collaboratively, to be proactive and to show initiative is essential for this role.

Salary information:

Hybrid working in London: £32,682 per year, increasing to £33,600 after 6 months and £34,493 after 12 months Pro-rata if part-time

Hillside Clubhouse is recruiting for a mental health employment specialist to join our dynamic Camden team supporting people with mental health conditions to gain and retain employment.  The role is working within the nationally recognised IPS (Individual Placement and Support) model within an NHS secondary care mental health team based in the heart of Camden.

Who we are: 

Hillside Clubhouse is a co-produced mental health employment support charity working across Camden and Islington. We support people with mental health conditions into training, volunteering, and employment opportunities. People can access our services via our Clubhouse, where they can share their skills and expertise with others and support the running of the charity.  Alternatively, they can receive support out in the community from one of our employment specialists based within primary and secondary care.

The Role: 

The role is within our IPS Service and will be based within a Camden NHS secondary care team. As an IPS Employment Specialist, you’ll play an important role in supporting people with lived mental health experience on their recovery journey by supporting them back into paid employment. This is an opportunity to offer ongoing support, to motivate and provide people with hope, while developing the tools and skills required to recover, ultimately transforming their lives.

A variety of skills are needed in this post including but not limited to -  a strong ability to develop empathy and rapport with clients; a determined approach to seeking out job opportunities; a person centric-approach to working with people; strong communications skills to motivate clients; strong administrative capabilities and an attention to detail engage with prospective employers via business development and face-to-face meetings, and to be able to challenge some perceptions around mental health. 

The postholder will be directly employed by Hillside Clubhouse but work under an Honorary Contract from Camden and Islington Foundation Trust.  They will be managed directly by the IPS Operations Manager but work as team member within the clinical team. The role is dynamic and involves being integrated in a clinical team, working within the community, and we operate a flexible working policy where working from home is also permitted up to two days per week.

Overall aim: 

To enable people with mental health issues to gain and sustain competitive employment by working within the evidence based Individual Placement and Support (IPS) model.

Primary Duties: 

• Manage a caseload of up to twenty clients and work within and apply the 8 principles of IPS in all aspects of role. 
• To work with and enable clients to find work using a mix of conventional approaches and employer engagement. 
• To generate ongoing referrals from the clinical team and to manage those effectively. 
• To engage directly and collaboratively with the clinical team while maintaining the existing multi-disciplinary approach to assisting clients seeking work. 
• To report and meet regularly with clinical team service managers and the IPS Operations manager, and to participate in both casework and clinical supervision from IPS Operations and clinical team services managers. 
• To be an active member of both the clinical and Hillside teams 
• To be responsible for own management of administrative duties tied to both Hillside Clubhouse and clinical team and adhering to existing Hillside and NHS protocols. 
• Source job opportunities specific to client needs from a range of sources including those advertised via current and conventional means, as well as through contact with local and Pan London employers. 
• To build up a network of employers to secure available and future vacancies. 
• To maintain existing and seek out new relationships with a range of external agencies of use to the post. 
• To provide ongoing support to both client and employers (where appropriate) when starting work and until such time as the client makes the decision to end the support provided. 
• To work with clients to collaboratively manage job searching activities and engagement with the service. 
• To manage and meet all KPIs, statistics and documentation as required, including but not limited to: referrals, first meetings, job outcomes, job sustainments of 6, 13 and 26 weeks, and work retention. 
• To provide guidance on the impact and/or the potential change to benefits when clients start work. 
• To work and manage time effectively in both community/external settings, and at clinical team.

• Secondary Duties: 
• To work out of hours as and when required, working in line with Hillside Clubhouse risk management procedures. 
• Undertake training when required and participate in ongoing learning and development. 
• To maintain professional boundaries with all internal and external stake holders including clients, and staff members of both Hillside and the clinical team. 
• To engage and participate in external IPS support services, training, and events. 
• To undertake additional tasks and duties as and when required, and to work outside of normal office hours when necessary.
